2. Or, pick up a Registration Form at the Kidz XL office anytime between 2 P.M. and 6 P.M., Monday through Friday.
3. Fill out the Registration Form completely.
4. Return the form to the Kidz XL office anytime between 2 P.M. and 6 P.M., Monday through Friday. Please bring all the necessary identification and records that are listed in the Registration Form when you come. The Kidz XL office is located on the second floor of Olympia Heights Methodist Church where Kidz XL is located. The address is:

If you mail your Registration Form, please call us at (305) 221-1404 to let us know that it is coming. You will need to bring all the necessary identification and records that are listed in the Registration Form when you child arrives at the first day of camp.
